Summary

This case study shows a successful surgical technique using Tenon graft to repair a phacoemulsification wound burn. The method ensured good wound healing and rapid visual recovery without astigmatism.

Background:

  • Phacomorphic angle closure with dense nuclear sclerosis presents surgical challenges during phacoemulsification.
  • Tight corneal wounds and phacoemulsification wound burn can complicate cataract surgery outcomes.

Observation:

  • A 75-year-old woman with challenging surgical conditions experienced a phacoemulsification wound burn.
  • A Tenon graft harvested from the conjunctiva was used to repair the corneal wound gape.
  • The graft was secured with two radial corneoscleral 10-0 nylon sutures.

Findings:

  • Anterior optical coherence tomography confirmed excellent wound apposition 10 days post-surgery.
  • Sutures were removed, and the patient achieved significant visual recovery to 20/25 (6/7.5).
  • No significant astigmatism was detected post-operatively.

Implications:

  • Tenon grafting is an effective method for managing phacoemulsification wound burns.
  • This technique facilitates rapid visual rehabilitation and minimizes post-operative astigmatism.
  • The approach offers a viable solution for complex cataract surgery cases.
