Summary

Gluteus maximus myocutaneous flaps effectively reconstruct sacral-gluteal, perineal, and trochanteric defects. This surgical technique demonstrated no flap necrosis, with good patient recovery and early mobilization.

Area of Science:

  • Plastic Surgery
  • Reconstructive Surgery

Background:

  • Gluteus maximus myocutaneous flaps are a reconstructive option for complex defects.
  • Over 40 years of clinical experience exists with this technique.

Purpose of the Study:

  • To evaluate the efficacy and outcomes of gluteus maximus myocutaneous flaps in defect reconstruction.
  • To analyze the application of classical and nontypical flap variations.

Main Methods:

  • Retrospective review of 42 patients treated with gluteus maximus myocutaneous flaps since 1984.
  • Classification of flaps into 'classical' (sacral-gluteal defects) and 'nontypical' (lumbar, perineal, trochanteric defects).

Main Results:

  • All 42 defects were successfully closed without flap necrosis.
  • Minor wound complications included infections (12 patients) and dehiscences (6 patients).
  • Average blood loss was <500 cc, with a 17-day average hospitalization and mobilization within 3-4 days postoperatively.

Conclusions:

  • Gluteus maximus myocutaneous flaps are a reliable and safe method for reconstructing various defects.
  • Both classical and nontypical flap designs offer successful closure with acceptable morbidity.
