Area of Science:

  • Urology
  • Gynecology
  • Neuroscience

Background:

  • Urinary incontinence during orgasm is a rarely reported symptom in women.
  • Understanding the physiological mechanisms is crucial for diagnosis and management.

Observation:

  • Three female patients experiencing orgasm-related urinary incontinence were studied.
  • Urodynamic assessments were conducted before and during sexual climax.

Findings:

  • During orgasm, patients exhibited involuntary bladder contractions.
  • Simultaneously, urethral relaxation occurred, leading to urine leakage.
  • These bladder contractions were absent during pre-orgasm urodynamic testing.

Implications:

  • Orgasm-induced incontinence appears multifactorial.
  • Potential causes include sphincter incompetence, neuropathic hyperreflexic bladder, and non-neuropathic idiopathic bladder instability.
  • Further research is needed to elucidate the precise pathways and develop targeted treatments.
