Summary

Clonidine, a medication for hypertension, can cause sinus node dysfunction, leading to dangerous heart pauses. Discontinuation of clonidine resolved these episodes in a patient with pre-existing conditions.

Area of Science:

  • Cardiology
  • Clinical Pharmacology

Background:

  • Clonidine is an antihypertensive agent.
  • Previous studies linked clonidine to atrioventricular (AV) node abnormalities.

Observation:

  • A 65-year-old male with diabetes, hypertension, and renal failure experienced dizziness and syncope after starting clonidine.
  • Continuous ECG monitoring revealed significant sinus bradycardia, nodal rhythm, and prolonged sinus arrest episodes.

Findings:

  • Clonidine use was associated with concentration-dependent sinus node dysfunction.
  • Discontinuation of clonidine led to the resolution of sinus arrest episodes.

Implications:

  • This case highlights a potential adverse effect of clonidine on sinus node function.
