Urinary Tract Calculi VI: Surgical Management

133
Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Summary

A spontaneous bladder rupture occurred in a 79-year-old man, presenting as bowel obstruction symptoms. Emergency laparoscopy revealed a bladder hole tamponaded by an ileum loop, highlighting urinary catheter use as a risk factor.

Area of Science:

  • Urology
  • Gastroenterology
  • Surgical Case Report

Background:

  • A 79-year-old male presented with lower abdominal pain and symptoms suggestive of bowel obstruction.
  • The patient had a history of urinary retention managed with an indwelling bladder catheter due to phimosis.

Observation:

  • Contrast-enhanced computed tomography revealed significant gastrectasia and small bowel distention, initially suspected to be caused by an adherent bridle.
  • Clinical signs and laboratory tests strongly indicated bowel obstruction, necessitating emergency surgical intervention.

Findings:

  • Exploratory laparoscopy unexpectedly identified a spontaneous bladder rupture, with an ileum loop tamponading the perforation.
  • The bladder perforation was successfully repaired laparoscopically.

Implications:

  • This case highlights spontaneous bladder rupture as a rare but critical diagnosis in patients with abdominal pain and urinary catheters.
  • Emergency laparoscopic surgery proved effective for both diagnosis and treatment of this condition.
  • The presence of an indwelling urinary catheter may represent a risk factor for intraperitoneal bladder rupture, warranting clinical consideration.
