Background:

  • Limited understanding exists regarding the link between plasma Aβ and subclinical white matter (WM) integrity in individuals at risk for Alzheimer's disease (AD).
  • This study focuses on individuals experiencing subjective cognitive decline (SCD), a prodromal stage of AD.

Purpose of the Study:

  • To investigate the association between plasma Aβ levels and microstructural changes in white matter (WM).
  • To explore the correlation between plasma Aβ40 and Aβ42 levels and WM integrity in individuals with SCD.
  • To determine if WM integrity differences correlate with cognitive performance.

Main Methods:

  • 142 cognitively normal individuals with SCD were recruited from the Sino Longitudinal Study on Cognitive Decline (SILCODE).
  • Participants underwent neuropsychological testing, plasma Aβ measurements (Aβ40 and Aβ42), and diffusion tensor imaging (DTI).
  • Tract-based spatial statistics (TBSS) were used to compare fractional anisotropy (FA) and mean diffusivity (MD) in WM between low and high plasma Aβ groups.

Main Results:

  • Individuals with high plasma Aβ40 levels exhibited significantly decreased FA and increased MD in multiple WM regions compared to those with low Aβ40.
  • These affected regions included the corpus callosum, superior longitudinal fasciculus, corona radiata, and others.
  • Lower average FA values were associated with poorer performance in executive function and memory assessments.
  • No significant differences in WM integrity (MD or FA) were observed between low and high plasma Aβ42 groups.

Conclusions:

  • Plasma Aβ40 levels are correlated with white matter (WM) integrity in individuals with subjective cognitive decline (SCD).
  • These findings suggest that plasma Aβ40 may serve as a potential biomarker for detecting early WM changes associated with Alzheimer's disease risk.
  • Further research is warranted to explore the clinical utility of plasma Aβ40 in predicting AD progression.
