  • The case highlights a rare neurological manifestation of scrub typhus: non-convulsive status epilepticus (NCSE).

Observation:

  • A 50-year-old female presented with fever and altered sensorium, later diagnosed with scrub typhus via IgM ELISA.
  • Despite initial treatment with doxycycline and ceftriaxone, her condition did not improve significantly.

Findings:

  • Electroencephalogram (EEG) confirmed generalized epileptiform discharges, indicative of NCSE, which responded to intravenous lorazepam.
  • Subsequent treatment with valproate and levetiracetam led to the patient regaining consciousness and achieving a full recovery.

  • This case demonstrates that patients with scrub typhus-induced NCSE can achieve complete recovery with appropriate neurological management.
  • Early recognition and prompt treatment of NCSE in scrub typhus patients are crucial for favorable outcomes.
