Summary
Inguinal hernias containing portosystemic shunt vessels are rare. Imaging like ultrasound and CT is crucial for safe surgical repair in patients with portal hypertension.

- A 72-year-old man presented with an indirect inguinal hernia containing an expanded portosystemic shunt vessel.
- Computed tomography (CT) confirmed the shunt vessel, previously noted due to liver cirrhosis.
Findings:
- Lichtenstein's herniorrhaphy was performed, identifying the shunt vessel within the hernia sac.
- Branches between the shunt vessel and testicular vein were carefully managed during repair.
Implications:
- Ultrasound is recommended for inguinal hernia patients with portal hypertension to identify atypical contents.
- CT imaging aids in planning safe surgical repair when shunt vessels are suspected.
