Summary

Obsessive-compulsive disorder (OCD) shows altered brain dynamics, with patients spending more time in a highly segregated network state linked to anxiety. This suggests impaired functional flexibility in key brain networks contributes to OCD neuropathology.

Background:

  • Obsessive-compulsive disorder (OCD) is a disabling neuropsychiatric condition with an unclear neurobiological basis.
  • Previous studies using static resting-state fMRI identified aberrant functional connectivity (FC) in OCD, but dynamic FC and network topology remain underexplored.

Purpose of the Study:

  • To investigate whole-brain dynamic functional connectivity (dFC) and network topological properties in patients with OCD.
  • To explore the relationship between aberrant brain dynamics, clinical symptoms, and illness duration in OCD.

Main Methods:

  • Resting-state fMRI data from 29 OCD patients and 40 healthy controls were analyzed.
  • A sliding-window approach generated dFC matrices, which were clustered into recurring brain states.
  • Graph theory methods assessed network topological organization variability and nodal efficiency within default mode (DMN) and salience networks (SAN).

Main Results:

  • OCD patients exhibited increased time in a specific brain state (State 1) characterized by high modularity and low inter-network FC.
  • Fractional time in State 1 correlated positively with anxiety levels in OCD patients.
  • Lower temporal variability in nodal degree centrality and efficiency within DMN and SAN was observed in OCD, linked to illness duration and obsession severity.

Conclusions:

  • Aberrant dynamic brain activity, including increased functional segregation and impaired functional flexibility within DMN and SAN, is evident in OCD.
  • These dynamic alterations may play a significant role in the neuropathology of obsessive-compulsive disorder.
