Background:

  • Complete post-traumatic paraplegia below the T6 level can lead to significant neurological and functional deficits.

Observation:

  • A patient with paraplegia experienced urinary problems and later developed syringomyelia.
  • The onset of micturition difficulties coincided with the initial sensory symptoms of syringomyelia.
  • The patient engaged in significant and repeated efforts to achieve reflex micturition.

Findings:

  • A potential association exists between the strain of reflex voiding and the pathogenesis of post-traumatic syringomyelia.
  • Repeated Valsalva maneuvers during attempted reflex micturition may increase intraspinal pressure.
  • This increased pressure could be a contributing factor to the development or progression of syringomyelia.

Implications:

  • Clinicians should consider the potential impact of voiding strategies on syringomyelia development in SCI patients.
  • Further research is warranted to elucidate the biomechanical link between detrusor overactivity and syringomyelia.
  • This highlights the importance of comprehensive management of bladder function post-SCI to prevent secondary complications.
