Background:

  • Coccidioidomycosis is a fungal infection endemic to the southwestern United States.
  • Eosinophilia, an elevated eosinophil count, can be a rare manifestation of coccidioidomycosis.
  • Diagnosis and management of disseminated coccidioidomycosis can be challenging.

Observation:

  • A case of coccidioidomycosis presented with pleurisy, skin lesions, and meningitis.
  • The patient exhibited significant eosinophilia in both peripheral blood and cerebrospinal fluid.
  • These clinical signs and laboratory findings indicated a severe disseminated infection.

Findings:

  • Prompt resolution of pleurisy, skin lesions, meningitis, and eosinophilia was observed.
  • Treatment with amphotericin B was initiated for the disseminated coccidioidomycosis.
  • The patient's response to amphotericin B was rapid and positive.

Implications:

  • Eosinophilia should be considered in the diagnostic workup of suspected coccidioidomycosis, especially with disseminated disease.
  • Cerebrospinal fluid eosinophilia may indicate central nervous system involvement in coccidioidomycosis.
  • Amphotericin B remains an effective treatment for severe and disseminated coccidioidomycosis.
