Summary

A novel artificial intelligence (AI)-aided polyp detection device, Skout, significantly increased the adenoma detection rate (ADR) during colonoscopies. This AI tool shows promise for improving patient outcomes in colorectal cancer screening and surveillance.

Area of Science:

  • Gastroenterology
  • Medical Technology
  • Artificial Intelligence in Medicine

Background:

  • High-quality colorectal cancer screening relies on detecting neoplasia, measured by adenoma detection rate (ADR) and adenomas per colonoscopy (APC).
  • Current colonoscopy performance can be enhanced through advanced technologies.

Purpose of the Study:

  • To evaluate the efficacy of a novel AI-aided polyp detection device, Skout, in routine colonoscopy.
  • To assess the impact of Skout on ADR and APC as primary endpoints.

Main Methods:

  • A prospective, single-arm, unblinded trial comparing colonoscopies with and without the AI device (Skout).
  • Results were compared against a historical cohort.
  • Histological examination confirmed all resected polyps.

Main Results:

  • The ADR was significantly higher with Skout (54.2%) compared to without (40.6%) overall (P=0.028).
  • For screening exams, ADR was 53.6% with Skout versus 30.8% without (P=0.024).
  • Adenomas per colonoscopy (APC) was significantly higher in screening exams with Skout (1.18) versus without (0.50) (P=0.002).

Conclusions:

  • The AI-aided polyp detection device (Skout) effectively increased ADR in colonoscopies.
  • The use of Skout did not lead to a significant increase in hyperplastic polyp resection.
  • AI-assisted colonoscopy presents a potential advancement for improving patient outcomes.
