Glaucoma: Overview

1.1K
Glaucoma is an eye condition characterized by increased intraocular pressure that damages the retina and optic nerve, leading to irreversible blindness if left untreated. The human eye has various components, including the cornea, iris, pupil, lens, and optic nerve. Aqueous humor is secreted by the epithelium of the ciliary body in the posterior chamber and flows through the trabecular meshwork and canal of Schlemm, maintaining normal intraocular pressure. The trabecular meshwork and the canal...

Open Angle Glaucoma: Treatment

823
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.
Drugs such as carbonic anhydrase inhibitors, α2- and...

Angle Closure Glaucoma: Treatment

952
Angle-closure glaucoma, or closed-angle glaucoma, is an eye condition where the iris bulges out and blocks the iridocorneal angle, resulting in a buildup of aqueous humor and increased intraocular pressure. Immediate medical attention is necessary due to the sudden onset of symptoms. The treatment for angle-closure glaucoma includes short-term and long-term approaches. Short-term treatment involves using eye drops like pilocarpine to lower intraocular pressure by increasing aqueous humor...

Summary

Intraocular pressure (IOP) slightly increased long-term after pars plana vitrectomy (PPV). Silicone oil tamponade was linked to IOP rise, and both IOP rise and open-angle glaucoma were more probable post-vitrectomy.

Area of Science:

  • Ophthalmology
  • Surgical Innovation
  • Glaucoma Research

Background:

  • Pars plana vitrectomy (PPV) is a common intraocular surgery.
  • Long-term effects of PPV on intraocular pressure (IOP) require further investigation.
  • Understanding IOP changes post-PPV is crucial for patient management.

Purpose of the Study:

  • To evaluate long-term changes in intraocular pressure (IOP) following pars plana vitrectomy (PPV).
  • To identify risk factors associated with IOP elevation after PPV.
  • To assess the incidence of open-angle glaucoma (OAG) after PPV.

Main Methods:

  • Retrospective cohort study comparing operated eyes to fellow eyes.
  • Exclusion criteria included prior scleral buckling and silicone oil emulsification.
  • Primary outcomes were significant IOP rise (>6.0 mmHg) and development of OAG.

Main Results:

  • Mean IOP increased significantly from baseline (13.53 mmHg) to final follow-up (16.52 mmHg) (P < 0.001).
  • Silicone oil (SO) tamponade was associated with a higher probability of long-term IOP rise (P = 0.028).
  • Open-angle glaucoma (OAG) occurred in 7.5% of patients, with no association to SO tamponade.

Conclusions:

  • Pars plana vitrectomy (PPV) leads to a slight long-term increase in intraocular pressure (IOP).
  • Silicone oil (SO) tamponade is a risk factor for sustained IOP elevation post-PPV.
  • Both IOP elevation and OAG are more likely to develop after vitrectomy compared to the fellow eye.
