Background:

  • Bilateral cochlear implantation (CI) is increasingly common for adults with severe to profound hearing loss.
  • Predicting speech recognition outcomes with a second CI is crucial for patient counseling and decision-making.

Purpose of the Study:

  • To assess the relationship between patient, hearing, and CI-specific factors and speech recognition outcomes in adults receiving a second CI.
  • To identify predictors of speech recognition performance with the second CI.

Main Methods:

  • Retrospective review of a prospectively maintained CI database from a tertiary academic center.
  • Analysis of speech recognition scores (consonant-nucleus-consonant and AzBio) at ≥12 months postimplantation in 102 adults with bilateral CIs.

Main Results:

  • Postimplantation speech recognition scores of the first CI were independently associated with second CI performance.
  • First CI scores explained 24.3% (CNC) and 40.3% (AzBio) of the variation in second CI scores.
  • 75.2% (CNC) and 65.9% (AzBio) of patients achieved scores with their second CI equivalent to or better than their first.

Conclusions:

  • The degree of speech recognition improvement with the first CI may predict outcomes with the second CI.
  • These findings offer preliminary evidence for setting patient expectations regarding second CI benefits.
  • While helpful for decision-making, counseling should acknowledge unexplained outcome variability.
