Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

828
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Area of Science:

  • Traditional Chinese Medicine
  • Acupuncture Techniques
  • Classical Chinese Medical Texts

Background:

  • Chapter 7 of the Lingshu (Miraculous Pivot) details the lateral needling technique.
  • Understanding and application of this technique for tendon "bi syndrome" remain areas of inquiry.
  • Tendon "bi syndrome" is often associated with obstructed or retarded qi activity.

Purpose of the Study:

  • To clarify the original meaning and application of the lateral needling technique.
  • To establish lateral needling as a classic manipulation for tendon "bi syndrome."
  • To explain the mechanism and therapeutic scope of the technique.

Main Methods:

  • Analysis of original texts, specifically the Lingshu (Miraculous Pivot).
  • Examination of the indication for "tendon bi syndrome."
  • Description of the filiform needle operation involving coordinated hand techniques (puncture and pressing).

Main Results:

  • Lateral needling technique is identified as a classic manipulation for tendon "bi syndrome."
  • The technique primarily addresses tendon spasticity due to qi obstruction or retardation.
  • It is categorized as a "mass-loosening" technique within the Neijing (Inner Classic).

Conclusions:

  • Lateral needling technique is a reliable acupuncture method for acute and chronic diseases.
  • Effective clinical applications include cervical spondylosis and lumbar tendon injuries.
  • The technique demonstrates a stable therapeutic effect in practice.
