Background:

  • Osteochondritis dissecans (OCD) typically affects a single joint.
  • Multiple joint involvement, especially asymmetric and sequential lesions, is exceptionally rare.

Observation:

  • A 15-year-old male presented with sequential OCD lesions in the left knee (medial femoral condyle, then femoral trochlea) and right knee (femoral trochlea).
  • The patient had a personal history of elbow OCD and a paternal history of bilateral knee OCD.

Findings:

  • The patient underwent multiple surgical interventions for recurrent OCD lesions.
  • One lesion remained stable for 17 months before surgical intervention was required.

Implications:

  • Early MRI detection and intervention are vital for patients with a history of multiple OCDs.
  • Genetic predisposition and familial history may play a role in the development of multiple OCDs.
