This study developed a valid and reliable questionnaire to identify patient non-adherence reasons in outpatient clinics. The tool assesses intrapersonal, provider-related, and socio-economic factors influencing medical order adherence.
Area of Science:
- Health Services Research
- Patient Adherence Studies
- Questionnaire Development and Validation
Background:
- Patient non-adherence to medical orders in outpatient settings compromises treatment effectiveness and increases healthcare costs.
- Understanding the root causes of non-adherence is crucial for improving patient outcomes.
- This research addresses the need for a validated tool to assess these reasons.
Purpose of the Study:
- To develop and validate a comprehensive questionnaire for identifying patient-reported reasons for non-adherence to physicians' medical orders.
- To establish the psychometric properties of the developed instrument.
Main Methods:
- A sequential exploratory mixed-method design was employed, integrating qualitative (literature review, content analysis of 19 interviews) and quantitative phases.
- The quantitative phase involved construct validity assessment using exploratory factor analysis (EFA) and reliability testing via Cronbach's alpha on 440 outpatients.
- Data analysis utilized MAXQDA 10 for qualitative and Stata 14 for quantitative data.
Main Results:
- An 18-item questionnaire was finalized after content validity index (CVI) and ratio (CVR) assessments.
- Exploratory factor analysis identified four key dimensions: intrapersonal-psychological, intrapersonal-cognitive, provider-related, and socio-economic reasons.
- The instrument demonstrated good reliability, with Cronbach's alpha coefficients ranging from 0.66 to 0.73 for the factors and an overall reliability of 0.70.
Conclusions:
- The developed 18-item questionnaire is a valid and reliable tool for assessing patient-perceived reasons for non-adherence in outpatient services.
- This instrument can aid healthcare providers in understanding and addressing barriers to medical order compliance.
- Findings support the use of this questionnaire to enhance clinical effectiveness and patient care in outpatient settings.
