Summary
Nasolabial cysts (NC) are rare soft tissue lesions. This case report details a 76-year-old woman with a 20-year history of an asymptomatic nasolabial cyst, successfully treated with excisional biopsy.

Background:
- Nasolabial Cyst (NC), also known as Klestadt's cyst, is a rare developmental lesion of the nasal alar region.
- It accounts for 0.7% of non-odontogenic cysts and typically presents as an asymptomatic swelling causing cosmetic concerns due to facial asymmetry.
Observation:
- A 76-year-old female presented with a 20-year history of an asymptomatic left-sided upper lip swelling.
- Clinical examination revealed a sessile, non-tender swelling in the upper labial mucosa near the left incisor region, causing facial asymmetry.
- Microscopic examination confirmed a cystic cavity lined by cuboidal to columnar epithelium with mucous cells and chronic inflammatory infiltrate.
Findings:
- The excisional biopsy of the nasolabial cyst was performed successfully.
- Histopathological analysis revealed characteristic features of a nasolabial cyst.
Implications:
- Clinicians should consider nasolabial cysts in the differential diagnosis of upper lip swellings, particularly in elderly patients.
- Prompt diagnosis and surgical excision ensure favorable outcomes with no recurrence, as demonstrated by the 2-year follow-up.
