Background:

  • Ocular manifestations are common in Human Immunodeficiency Virus (HIV) infection.
  • The prognostic significance of these eye conditions and their link to immune status requires further elucidation.

Purpose of the Study:

  • To longitudinally assess the prognostic value of ocular manifestations in HIV-infected individuals.
  • To investigate the correlation between ocular changes and immune system alterations, specifically CD4+ lymphocyte counts.

Main Methods:

  • Longitudinal study involving 115 HIV-infected subjects (75 PGL, 23 ARC, 17 AIDS).
  • Average follow-up period of one year (3-22 months).
  • Ophthalmoscopic examinations to identify ocular manifestations, particularly retinal cotton-wool-like spots.

Main Results:

  • Retinal cotton-wool-like spots were the most frequent ocular finding (58.8% in AIDS, 76.9% with ocular involvement).
  • Two of three ARC patients with cotton-wool spots developed Pneumocystis pneumonia (PCP) shortly after examination.
  • A strong correlation was observed between ocular changes and a decrease in CD4+ lymphocyte counts.

Conclusions:

  • Ocular manifestations in HIV infection are closely linked to immune system deterioration.
  • Eye findings, such as cotton-wool spots, can be as informative as opportunistic infections or neoplasms for predicting HIV prognosis.
