Summary

Near-infrared autofluorescence (NIRAF) imaging reveals a distinct autofluorescent cap in most parathyroid adenomas. This cap, representing normal parathyroid tissue, aids in identifying abnormal glands during surgery for primary hyperparathyroidism.

Background:

  • Primary hyperparathyroidism (pHPT) is a common endocrine disorder, frequently caused by parathyroid adenomas.
  • Near-infrared autofluorescence (NIRAF) is a developing technique for parathyroid tissue identification in surgery.
  • Current consensus is lacking on NIRAF's ability to distinguish normal from abnormal parathyroid glands.

Purpose of the Study:

  • To characterize the autofluorescence pattern of parathyroid adenomas in patients with pHPT.
  • To assess the utility of NIRAF in differentiating parathyroid adenomas from surrounding normal tissue.

Main Methods:

  • Patients with pHPT undergoing parathyroid adenoma resection were enrolled.
  • Parathyroid autofluorescence was measured using NIRAF imaging during surgery.
  • Histological confirmation of parathyroid adenomas was performed.

Main Results:

  • Parathyroid adenomas exhibited a heterogeneous autofluorescence pattern.
  • A well-defined, more fluorescent 'cap' region was observed in 17 out of 23 adenomas.
  • This cap region corresponded to normal parathyroid tissue, showing 88% sensitivity and 67% specificity.

Conclusions:

  • Parathyroid adenomas display a heterogeneous autofluorescence pattern with NIRAF.
  • A distinct autofluorescent portion, indicative of normal parathyroid tissue, is frequently identified.
  • Further research is needed to validate these NIRAF imaging findings in distinguishing parathyroid adenomas.
