Summary

Posterior uveal effusion (UE) can occur after cataract surgery. Long-term observation showed normal macular morphology and improved vision, suggesting observation may be suitable for some resistant cases.

Background:

  • Cataract surgery, specifically phacoemulsification with intraocular lens implantation, can rarely lead to posterior uveal effusion (UE).
  • Posterior UE is characterized by fluid accumulation in the choroid and retina, potentially causing vision impairment.

Observation:

  • A 64-year-old woman developed diminished vision in her right eye three weeks post-cataract surgery.
  • Ophthalmic examinations, including OCT and angiography, revealed choroidal folds, subretinal fluid, and choroidal darkening.
  • Initial treatment with anti-inflammatory and systemic corticosteroids for five months showed no morphological improvement.

Findings:

  • After a long-term follow-up (43 months post-surgery), optical coherence tomography (OCT) demonstrated normal macular morphology.
  • Best corrected visual acuity (BCVA) improved to 20/25 without ongoing treatment, indicating spontaneous resolution.
  • OCT proved to be a reliable tool for monitoring macular changes in posterior UE.

Implications:

  • Posterior uveal effusion is a rare but possible complication following modern cataract surgery.
  • Long-term observation may be a viable management strategy for selected cases of posterior UE resistant to conventional therapy.
  • This case highlights the potential for spontaneous recovery and functional improvement in posterior UE over time.
