Leukocyte response and hypoglycemia in superlethal endotoxic shock.

G L White, L T Archer, B K Beller

    Summary

Leukocyte levels impact survival in endotoxin shock. Initial leukocytosis (high white blood cell count) protected dogs from lethal hypoglycemia and death, suggesting a key role for these cells in endotoxin shock survival.

    Area of Science:

    • Immunology
    • Pathophysiology
    • Veterinary Medicine

    Background:

    • Endotoxin shock in dogs causes hypoglycemia, hypotension, and death.
    • Accelerated glucose uptake post-endotoxin is linked to buffy coat components.
    • Leukocytes may play a protective role against endotoxin's lethal effects.

    Purpose of the Study:

    • To investigate the protective role of leukocytes against lethal endotoxin shock in awake dogs.
    • To correlate leukocyte response with survival and glucose levels during endotoxemia.

    Main Methods:

    • Paired groups of awake dogs: saline controls and endotoxin-treated.
    • Endotoxin administration: escalating doses over 4 days.
    • Monitoring: body temperature, leukocyte counts, and blood glucose levels.

    Main Results:

    • Endotoxin induced fever, initial leukopenia, and subsequent leukocytosis.
    • Dogs with initial leukocytosis survived superlethal endotoxin doses without lethal hypoglycemia.
    • Dogs with normal initial leukocyte counts died with severe hypoglycemia (mean 40 mg%).

    Conclusions:

    • Initial leukocytosis is associated with improved survivability in endotoxin shock.
    • Sustained gluconeogenic function appears critical for surviving endotoxin challenge.
    • Leukocyte response is a significant factor in the pathophysiology of endotoxin shock.
