Research on degenerative cervical myelopathy (DCM) is growing, but current studies do not fully align with established priorities. Future research should address under-explored areas to advance understanding and treatment of this common neurodegenerative disorder.

Background:

  • Degenerative cervical myelopathy (DCM) is a prevalent, progressive neurodegenerative condition affecting 2% of adults.
  • DCM significantly diminishes patients' quality of life.
  • The AO Spine RECODE-DCM project identified key research priorities for the field.

Purpose of the Study:

  • To assess the alignment of current research activity with the established AO Spine RECODE-DCM research priorities.
  • To identify research gaps and imbalances in the study of degenerative cervical myelopathy.

Main Methods:

  • A systematic review of MEDLINE and Embase databases was performed for studies on cervical myelopathy.
  • Data from 2261 eligible papers published between 1995 and 2020 were analyzed.
  • Studies were evaluated for their alignment with the identified research priorities.

  • A total of 2261 papers and 1,323,979 patients were included in the review.
  • While 89% of papers aligned with at least one priority, research focus was unequal.
  • Priority #15 (predictors of outcome) was heavily researched (1060 papers), while 10 other priorities received minimal attention (64 papers total).

  • Research in degenerative cervical myelopathy is expanding, with a continued emphasis on surgical aspects.
  • The established research priorities offer a new, necessary direction for the field.
  • Addressing under-researched priorities is crucial for advancing DCM knowledge and patient care.
