New-Onset Diabetes in Children during COVID-19: Clinical Case Report

COVID-19 can present atypically in children, sometimes revealing itself through new-onset diabetes and diabetic ketoacidosis. Early suspicion in pediatric cases with hyperglycemia is crucial for timely management.

Observation:
- A 3-year-old child presented with loss of consciousness, lethargy, and stupor, without fever.
- Clinical assessment revealed dehydration, tachypnea, and altered consciousness (Glasgow Coma Scale 10/15).
- Laboratory tests showed hyperglycemia (5 g/L), lymphopenia, glycosuria, ketonuria, and elevated HbA1c (10%).
Findings:
- The child tested positive for COVID-19 via RT-PCR.
- The presentation mimicked new-onset diabetes with diabetic ketoacidosis.
Implications:
- COVID-19 should be considered in children presenting with atypical symptoms like new-onset diabetes and diabetic ketoacidosis.
- Prompt recognition of hyperglycemia and elevated HbA1c in conjunction with COVID-19 suspicion can facilitate early patient management.
