Routine colonoscopy after uncomplicated right colon diverticulitis is recommended. While colon cancer is rare, this follow-up can detect advanced adenomas in younger patients.
Area of Science:
- Gastroenterology
- Colorectal Surgery
- Oncology
Background:
- Colon cancer screening guidelines often recommend colonoscopy after diverticulitis treatment.
- The necessity of routine colonoscopy for uncomplicated diverticulitis, particularly right-sided, remains debated.
- Right colon diverticulitis affects younger individuals, necessitating specific evaluation of follow-up strategies.
Purpose of the Study:
- To assess the incidence of right colon cancer and adenomatous polyps after conservative management of acute uncomplicated right colon diverticulitis.
- To evaluate the clinical relevance of routine colonoscopy surveillance in this patient population.
Main Methods:
- Retrospective analysis of 330 patients with uncomplicated right colon diverticulitis (modified Hinchey stage Ia) diagnosed via CT scan between 2011-2017.
- Inclusion criteria required follow-up colonoscopy after treatment.
- Primary outcome: colon cancer incidence; Secondary outcome: adenoma detection rate.
Main Results:
- Colon cancer was detected in 0.3% (1/330) of patients.
- Adenoma detection rate was 20.9% (69/330).
- Advanced adenomas (including cancer) were found in 2.7% (9/330) of patients, with a mean age of 41.9 years.
Conclusions:
- Routine colonoscopy following acute uncomplicated right colon diverticulitis is warranted.
- Despite a low colon cancer detection rate, follow-up colonoscopy can identify significant advanced adenomas.
- This strategy is particularly relevant for younger patients diagnosed with right-sided diverticulitis.
