Summary

Acetabular fracture outcomes can be poor. Femoral head damage, dislocation, and poor fracture reduction are key predictors for needing total hip arthroplasty (THA) after surgery.

Background:

  • Acetabular fractures can lead to poor functional outcomes in up to 25% of cases.
  • Predictors for total hip arthroplasty (THA) after surgical treatment of acetabular fractures require further analysis.

Purpose of the Study:

  • To identify predictors associated with poor outcomes and the need for THA in surgically treated acetabular fractures.

Main Methods:

  • Retrospective study of 207 patients with acetabular fractures over 14 years.
  • Analysis of demographic data, fracture patterns (AO/OTA), surgical complications, and predictor variables.

Main Results:

  • Seagull sign, femoral head dislocation, osteochondral damage, acetabular impaction, poor post-operative roof congruency, and congruence >3mm predicted THA.
  • Age, sex, AO-OTA classification, dislocation type, surgical approach, time to surgery, and posterior wall involvement did not predict THA.
  • Nerve injury was the most frequent complication (20.18%); 15.6% of patients required THA at an average of 6 years post-surgery.

Conclusions:

  • Femoral head damage and dislocation are significant predictors for THA.
  • Fracture reduction quality and the presence of a seagull sign are strong indicators for THA following acetabular fracture surgery.
