Background:

Polycystic ovary syndrome (PCOS) is a common endocrine disorder in reproductive-aged women.
Oral contraceptives are frequently used for managing PCOS symptoms.
Persistent right iliac fossa pain and jaundice warrant thorough investigation.

Observation:

The patient presented with a nine-month history of right iliac fossa pain, hyporexia, and conjugated hyperbilirubinemia.
Laboratory results showed elevated total and conjugated bilirubin levels.
Abdominal computed tomography (CT) demonstrated homogeneous hepatosplenomegaly and adenopathies in both iliac chains.

Findings:

The largest adenopathy measured 1.6 x 3.6 cm in the right external iliac chain.
Hepatosplenomegaly indicates potential systemic involvement or secondary effects.
Iliac adenopathies suggest possible inflammation, infection, or malignancy.

Implications:

The findings necessitate further diagnostic workup to determine the etiology of the hepatosplenomegaly and adenopathies.
Differential diagnoses may include infectious, inflammatory, or neoplastic processes.
Understanding the cause is crucial for appropriate management and treatment of the patient's symptoms.
