A single course of teplizumab treatment significantly delayed type 1 diabetes (T1D) diagnosis and improved beta cell function in high-risk individuals. This immune therapy showed lasting metabolic benefits and was linked to specific CD8+ T cell subset changes.
Area of Science:
- Immunology
- Endocrinology
- Diabetes Research
Background:
- Type 1 diabetes (T1D) is an autoimmune disease characterized by the destruction of pancreatic beta cells.
- Nondiabetic relatives of T1D patients are at high risk for developing the disease.
- Teplizumab is an immunotherapy agent that targets T cells.
Purpose of the Study:
- To analyze the long-term effects of a single 14-day course of teplizumab on metabolic function and immune cells in high-risk individuals for T1D.
- To evaluate the impact of teplizumab on time to T1D diagnosis and beta cell function.
- To investigate the relationship between changes in T cell subsets and clinical response to teplizumab.
Main Methods:
- Extended follow-up of a randomized controlled trial involving nondiabetic relatives at high risk for T1D.
- Analysis of metabolic parameters including glucose tolerance, C-peptide area under the curve (AUC), and insulin secretory rates.
- Characterization of T cell subsets and their function, specifically focusing on CD8+ T cells.
Main Results:
- Teplizumab treatment significantly delayed the median time to T1D diagnosis (59.6 months vs. 27.1 months for placebo).
- Fifty percent of teplizumab-treated participants remained diabetes-free compared to 22% of placebo-treated participants.
- Teplizumab improved beta cell function, evidenced by increased C-peptide AUC, and this improvement was associated with an increase in partially exhausted memory CD8+ T cells.
Conclusions:
- A single course of teplizumab has lasting effects in delaying T1D diagnosis and improving beta cell function in high-risk individuals.
- Changes in specific CD8+ T cell subsets, particularly partially exhausted effector cells, are associated with the clinical response to teplizumab.
- Teplizumab represents a promising immune therapy for delaying the onset of T1D and improving metabolic outcomes.
