Summary

An extremely rare coronary artery anomaly, where the right coronary artery originates from the anterior descending artery, was found in two patients with coronary atherosclerotic disease and acute coronary syndrome. This case highlights a unique presentation of single left coronary artery.

Background:

Anomalous origin of coronary arteries is a rare congenital condition.
The origin of the right coronary artery from the anterior descending artery is exceptionally uncommon.
Coronary artery anomalies can predispose individuals to myocardial ischemia and sudden cardiac death.

Observation:

Two patients with coronary atherosclerotic disease and acute coronary syndrome presented with a single left coronary artery.
The right coronary artery originated from the middle third of the left anterior descending artery in both cases.
Clinical presentation included symptoms consistent with acute myocardial infarction.

Findings:

Detailed clinical and angiographic characteristics of these rare cases were documented.
The anatomical variation involved a single coronary ostium arising from the aorta, supplying both ventricles via a single trunk.
Atherosclerotic disease progression within the anomalous vessel contributed to ischemic events.

Implications:

This case report expands the understanding of coronary artery anomalies and their potential clinical manifestations.
Recognition of such anomalies is crucial for accurate diagnosis and appropriate management of ischemic heart disease.
Further research into the embryological basis and long-term outcomes of this specific anomaly is warranted.
