Summary
Tuberculosis epididymo-orchitis is a rare form of extrapulmonary tuberculosis. Early diagnosis and treatment are crucial to prevent testicular damage and preserve fertility.

- Tuberculosis epididymo-orchitis, an uncommon manifestation of extrapulmonary TB, specifically affects the epididymis and testicles.
Observation:
- A 37-year-old male presented with left testicular involvement.
- Ultrasound revealed a heterogeneous parenchymal echo with multiple ill-defined hypo-echoic areas in the left testicle.
- Color Doppler ultrasound showed hyperemia in the left testicle and bilateral epididymis.
Findings:
- The ultrasound findings strongly suggested extrapulmonary tuberculosis.
- A 6-month anti-TB treatment regimen, including rifampin, isoniazid, pyrazinamide, and ethambutol, led to significant symptom improvement.
Implications:
- Prompt diagnosis and treatment of TB epididymo-orchitis are essential.
- Early intervention can prevent the need for epididymectomy and preserve male fertility.
