This study presents a novel technique for partial ear reconstruction using tissue expansion, porous polyethylene framework, and a radial artery free flap. The combined approach yielded satisfactory results in a patient with squamous cell carcinoma. Keywords: ear reconstruction, squamous cell carcinoma, tissue expansion, porous polyethylene, radial artery free flap.
Area of Science:
- Plastic Surgery
- Reconstructive Surgery
- Otolaryngology
Background:
- Squamous cell carcinoma resection can lead to significant auricular defects.
- Previous ear reconstruction methods have limitations.
- Aesthetic and functional restoration of the ear is crucial for patient quality of life.
Observation:
- A 76-year-old male patient with a defect of the superior half of the right ear post-squamous cell carcinoma resection was treated.
- No prior reconstruction attempt was made.
- The defect involved substantial loss of the ear structure.
Findings:
- A novel three-stage reconstruction was performed: suprafascial tissue expansion, implantation of a porous polyethylene framework, and coverage with a radial artery free flap.
- The patient reported high satisfaction with the aesthetic and functional outcome at the 1-year follow-up.
- The combined technique demonstrated successful restoration of the ear's form and contour.
Implications:
- This combined approach offers a viable and effective solution for complex partial ear defects.
- It highlights the potential of integrating tissue expansion, alloplastic materials, and free flaps for auricular reconstruction.
- Further investigation into this technique may advance reconstructive surgery for head and neck defects.
