Summary

Exfoliative dermatitis (ED), a severe skin condition, can be caused by fungal infections like dermatophytosis. This case study shows successful treatment of ED due to dermatophytosis using antifungal medications.

Background:

  • Exfoliative dermatitis (ED), or erythroderma, is a critical skin condition characterized by erythema and scaling affecting over 90% of the body surface.
  • It can stem from various systemic and cutaneous diseases, including superficial fungal infections like dermatophytosis.

Observation:

  • A 39-year-old male presented with diffuse erythematous macules and scales across nearly his entire body, diagnosed as dermatophytosis.
  • Diagnosis was confirmed via direct microscopy, fungal culture, and histopathology.

Findings:

  • The patient received a treatment regimen including 2% ketoconazole shampoo and two monthly pulses of 200 mg itraconazole twice daily.
  • Significant clinical improvement, marked by reduced erythema and scaling, was observed by day 7.
  • Complete mycological and clinical resolution was achieved by day 29.

Implications:

  • This case highlights dermatophytosis as a potential, albeit rare, cause of exfoliative dermatitis.
  • Accurate etiological diagnosis is crucial for effective treatment of ED.
  • Antifungal therapy can successfully manage ED secondary to dermatophytosis.
