Background:

  • Carotid near-occlusion is a challenging diagnosis often overlooked in routine computed tomography angiography (CTA) assessments.
  • Accurate identification of carotid near-occlusion is crucial for preventing stroke and guiding treatment decisions.

Purpose of the Study:

  • To evaluate the diagnostic utility of phase-contrast magnetic resonance (PC-MR) imaging for detecting carotid near-occlusion.
  • To compare blood flow parameters between carotid near-occlusion and conventional significant carotid stenosis.

Main Methods:

  • Phase-contrast MR imaging was performed on 9 patients with carotid near-occlusion and 20 patients with conventional ≥50% carotid stenosis (mean 65% stenosis).
  • Mean internal carotid artery (ICA) blood flow was quantified and compared between the two groups.

Main Results:

  • Mean ICA flow was significantly lower in near-occlusion cases (52 mL/min) compared to conventional stenosis (198 mL/min, P < .001).
  • An ICA flow threshold of ≤110 mL/min demonstrated 100% sensitivity and specificity for diagnosing carotid near-occlusion.
