Summary

Transaxillary gas-free surgery offers an effective minimally invasive option for Zenker's diverticulectomy. This approach provides excellent surgical visualization and a better cosmetic outcome for patients.

Observation:

  • A case study of a 64-year-old patient with a large Zenker's diverticulum (6 cm) is presented.
  • Transaxillary gas-free minimally invasive diverticulectomy was successfully performed using an endoscopic linear stapler.
  • The procedure had a surgery time of 137 minutes with no postoperative complications.

Findings:

  • The transaxillary gas-free approach demonstrated effectiveness and feasibility.
  • Postoperative recovery was rapid, with no anastomotic leakage or recurrent laryngeal nerve injury.
  • The patient was discharged on postoperative day 7.

Implications:

  • This minimally invasive technique offers advantages such as clear surgical field visualization, precise manipulation, and improved cosmesis.
  • It may serve as a viable alternative to traditional and endoscopic diverticulectomy for select patients.
  • Further prospective studies and experience accumulation are necessary to validate this approach.
