Donanemab treatment improved cognitive and functional abilities in early Alzheimer's disease patients compared to placebo. Further research is needed to confirm donanemab's efficacy and safety for Alzheimer's disease.
Area of Science:
- Neurology
- Immunotherapy
- Neurodegenerative Diseases
Background:
- Alzheimer's disease (AD) is characterized by amyloid-beta (Aβ) peptide accumulation.
- Donanemab is an investigational antibody targeting modified deposited Aβ for early AD treatment.
Purpose of the Study:
- To evaluate the efficacy and safety of donanemab in early symptomatic Alzheimer's disease.
- To assess donanemab's impact on cognitive and functional measures and amyloid/tau burden.
Main Methods:
- Phase 2 trial with 257 early AD patients with confirmed tau and amyloid deposition.
- Randomized 1:1 assignment to intravenous donanemab or placebo every 4 weeks for up to 72 weeks.
- Primary outcome: change in Integrated Alzheimer's Disease Rating Scale (iADRS) score at 76 weeks.
Main Results:
- Donanemab showed a statistically significant improvement in the primary iADRS score compared to placebo (difference of 3.20; P=0.04).
- Secondary outcome measures showed mixed results, with no substantial differences in most.
- Donanemab significantly reduced amyloid plaque levels and global tau load, with reported amyloid-related imaging abnormalities.
Conclusions:
- Donanemab demonstrated improved composite scores for cognition and daily living activities versus placebo in early AD.
- Results for secondary outcomes were mixed, necessitating further investigation.
- Larger, longer trials are required to fully establish donanemab's efficacy and safety profile in Alzheimer's disease.
