Summary

A novel polymeric self-enclosable external support (SES) prevents vein graft stenosis by adapting micropore size to vessel dilation. This promotes vascular healing and reinforces veins, improving outcomes for vein-to-artery bypass surgery.

Area of Science:

  • Biomaterials Science
  • Vascular Surgery
  • Regenerative Medicine

Background:

  • Minimally invasive medical devices utilize shape memory materials.
  • Organ-movement-specific microscale shape programming is a significant unmet need.
  • Vein-to-artery grafting often leads to vein dilation and stenosis.

Purpose of the Study:

  • To design a polymeric self-enclosable external support (SES) for vascular grafts.
  • To enable microscale shape transition in response to vessel dilation.
  • To prevent stenosis and improve outcomes in vein-to-artery grafting.

Main Methods:

  • Developed a polymeric SES to wrap the vascular out-wall.
  • Programmed micropores to change size and interconnection upon dilation.
  • Utilized SES elastic shape-fixity to maintain vein dilation and control shape transition.

Main Results:

  • SES micropore architecture transitioned upon dilation, inducing beneficial inflammation.
  • Regeneration of vasa vasorum and smooth muscle cell migration to adventitia were observed.
  • Vein reinforcement and prevention of stenosis were achieved, rescuing ischemic disorders.

Conclusions:

  • The SES approach enables stable microscale shape transition, maintaining vein dilation.
  • This strategy promotes venous maturation and reinforces veins, preventing stenosis.
  • The SES is a game-changer for vein-to-artery grafting, promoting arterialization and preventing complications.
