Background:

  • Post-craniotomy intracranial haemorrhage can present atypically.
  • Remote cerebellar haemorrhage (RCH) is a rare complication following supra-tentorial craniotomies.

Observation:

  • The 'Zebra Sign' on imaging, characterized by alternating hyperdense (blood) and hypodense (normal parenchyma) stripes along cerebellar folia, is a key diagnostic indicator.

Findings:

  • RCH typically does not necessitate surgical intervention.
  • Surgical treatment for RCH is reserved for cases with significant hydrocephalus or neurological decline.

Implications:

  • Recognizing RCH and its characteristic 'Zebra Sign' can prevent unnecessary investigations like biopsies.
  • Clinical monitoring and serial imaging are often sufficient for managing RCH.
