Summary

Acute type A aortic dissection (AAAD) can cause upper limb ischemia. A rare case shows persistent ischemia after repair due to flap extension into the brachial artery, leading to amputation.

Background:

  • Acute type A aortic dissection (AAAD) is a life-threatening condition with significant surgical risks.
  • Distal propagation of the dissection flap can compromise blood flow to the upper extremities, causing limb ischemia.

Observation:

  • A 67-year-old male with AAAD developed persistent upper limb malperfusion post-repair.
  • Despite adequate surgical intervention, the dissection flap extended into the brachial artery, causing thrombosis.

Findings:

  • The patient underwent brachial artery embolectomy without successful revascularization.
  • Ultimately, an above-elbow amputation was required due to unresolved ischemia.
