Transcatheter aortic valve replacement (TAVR) is associated with stable or improved kidney function in most patients. Improved kidney function post-TAVR correlates with lower mortality, while deterioration increases mortality risk.
Area of Science:
- Cardiology
- Nephrology
- Interventional Cardiology
Background:
- Chronic kidney disease (CKD) is common in patients undergoing transcatheter aortic valve replacement (TAVR).
- Limited data exist on TAVR's impact on kidney function and subsequent mortality.
- Understanding cardiorenal interactions post-TAVR is crucial.
Purpose of the Study:
- To evaluate changes in kidney function after TAVR.
- To assess the association between kidney function changes and midterm mortality following TAVR.
Main Methods:
- Retrospective cohort study of 894 patients undergoing TAVR.
- Kidney function assessed by estimated glomerular filtration rate (eGFR) at baseline and 1 month post-TAVR.
- Mortality assessed at 2-year follow-up.
Main Results:
- Over 80% of patients had stable or improved kidney function (eGFR change ≥10%) at 1 month post-TAVR.
- A 10% or greater eGFR deterioration was linked to a 2.16-fold increased risk of 2-year mortality.
- Improved kidney function post-TAVR was associated with similar 2-year mortality to those with baseline good kidney function.
Conclusions:
- Kidney outcomes after TAVR are generally favorable, with most patients maintaining or improving renal function.
- Post-TAVR kidney function improvement is linked to reduced mortality.
- Cardiorenal syndrome may be a significant factor, with TAVR offering potential for both cardiac and renal function improvement.
