  • Verrucous epidermal nevi (VEN) are benign congenital hamartomas composed of keratinocytes.
  • Histological features typically include hyperkeratosis, acanthosis, and papillomatosis.
  • Epidermolytic hyperkeratosis (EHK) is a less common histological finding in VEN.

Observation:

  • A 6-year-old boy presented with bilaterally symmetrical linear epidermal nevi.
  • The nevi followed Blaschko's lines, a characteristic dermatomal pattern.
  • Histological examination revealed features of epidermolytic hyperkeratosis.

Findings:

  • The patient's verrucous epidermal nevi exhibited epidermolytic hyperkeratosis.
  • Treatment with topical keratolytics and emollients resulted in significant clinical improvement.
  • This represents the first reported case of VEN with EHK from Pakistan.

Implications:

  • This case expands the clinical and pathological spectrum of verrucous epidermal nevi.
  • It underscores the importance of considering EHK in the differential diagnosis of VEN.
  • The successful management with topical agents offers a potential therapeutic approach for similar cases.
