  • Genetic testing identified a mutation in the CDC73 gene.

Findings:

  • The patient underwent successful parathyroidectomy, normalizing calcium and PTH levels.
  • The CDC73 mutation is linked to hyperparathyroidism-jaw tumor syndrome, familial isolated hyperparathyroidism, and parathyroid carcinoma.
  • Despite the mutation, the patient showed no evidence of associated tumors on screening.

Implications:

  • This case highlights the importance of genetic screening for young patients diagnosed with unexplained primary hyperparathyroidism.
  • Identifying CDC73 mutations can inform genetic counseling and long-term surveillance for inheritable neoplasia.
  • Early genetic diagnosis can guide management and risk assessment in affected families.
