Summary

Acupuncture shows promise for treating post-stroke depression (PSD), demonstrating greater effectiveness and a better safety profile than antidepressants. However, more high-quality research is needed to confirm these findings for acupuncture in PSD patients.

Background:

  • Post-stroke depression (PSD) is a common complication affecting patient recovery.
  • The efficacy and safety of acupuncture for PSD are under investigation.
  • Existing evidence from randomized clinical trials (RCTs) requires synthesis.

Purpose of the Study:

  • To systematically review and synthesize evidence from RCTs on acupuncture for PSD.
  • To assess the effectiveness and safety of acupuncture compared to conventional treatments and antidepressants.

Main Methods:

  • Comprehensive literature search of RCTs in seven databases up to April 2020, with a supplementary search for recent RCTs.
  • Data extraction by two independent reviewers and meta-analysis using RevMan 5.3.
  • Quality of evidence appraised using the Grades of Recommendation, Assessment, Development and Evaluation (GRADE) system.

Main Results:

  • Seventeen RCTs involving 1402 patients were analyzed.
  • Acupuncture combined with conventional treatments significantly reduced depression scores (HAM-D) compared to conventional treatment alone.
  • Acupuncture showed a better safety profile than antidepressants, with fewer adverse events.

Conclusions:

  • Acupuncture appears to be more effective and safer than antidepressants for treating PSD.
  • The current evidence is of low to very low quality due to heterogeneity.
  • High-quality research is necessary to validate the therapeutic role of acupuncture in managing PSD.
