Summary

Teleneurology accurately diagnoses cerebrovascular disease (CVD), with a high sensitivity. However, specificity was lower, indicating a tendency for false positives. Missed thrombolysis candidates were rare.

Background:

  • Accurate diagnosis of acute cerebrovascular disease (CVD) via teleneurology is not well-established.
  • Characteristics of misdiagnosed patients in teleneurology require further investigation.

Purpose of the Study:

  • To evaluate the diagnostic accuracy of teleneurology for acute cerebrovascular disease.
  • To identify characteristics of patients misdiagnosed by teleneurology.

Main Methods:

  • A random sample of 1500 teleneurological consultations were screened.
  • Teleneurological diagnoses were compared with hospital discharge diagnoses.
  • Diagnostic accuracy metrics (sensitivity, specificity) were calculated for cerebrovascular disease (CVD).

Main Results:

  • Out of 1078 consultations, 52% were diagnosed with CVD.
  • Teleneurology demonstrated high sensitivity (95.2%) for CVD identification.
  • Specificity was 77.4%, indicating a tendency for false-positive diagnoses.

Conclusions:

  • Teleneurological consultations are accurate for identifying patients with CVD.
  • The rate of missed candidates for thrombolysis was very low (0.2%).
  • Misdiagnosed patients (stroke chameleons) showed similar characteristics to correctly diagnosed CVD patients, except for a lower prevalence of ischemic heart disease.
