Summary

Pain sensitivity in the retroauricular regions (RAR) is linked to idiopathic Bell's palsy (IBP) severity. Lower pain thresholds indicate milder IBP, while pain preceding facial weakness suggests more severe cases.

Background:

  • Non-motor symptoms, including heightened sensitivity, are common in idiopathic Bell's palsy (IBP).
  • Retroauricular region (RAR) pain is a notable non-motor symptom in IBP patients.

Purpose of the Study:

  • To investigate the relationship between pain threshold in the RAR and IBP severity.
  • To determine if the timing of RAR pain occurrence correlates with IBP severity.

Main Methods:

  • A study involving 220 participants (142 IBP patients, 78 healthy subjects).
  • IBP severity was assessed using the House-Brackmann and Sunnybrook Grading Scales.
  • Pain thresholds in the RAR were measured using a digital pressure algometer.

Main Results:

  • IBP patients in severity groups II-V exhibited lower bilateral RAR pain thresholds compared to healthy subjects and those in group VI.
  • No significant difference in pain threshold was observed between the affected and unaffected RAR in IBP patients.
  • Pain preceding paralysis was less common in milder IBP (groups II-III) but more frequent in severe IBP (groups V-VI), whereas pain occurring after paralysis onset was more common in milder cases.

Conclusions:

  • A lower pain threshold in the RAR is inversely associated with IBP severity.
  • Retroauricular pain occurring before facial weakness onset may indicate higher IBP severity, while pain after onset suggests lower severity.
