Summary

Unicompartmental knee arthroplasty (UKA) shows higher function scores and patient satisfaction compared to total knee arthroplasty (TKA) for osteoarthritis. UKA offers comparable safety with improved outcomes in the short term.

Background:

  • Unicompartmental knee arthroplasty (UKA) is a viable treatment for unicompartmental osteoarthritis (OA), offering potential benefits like natural knee kinematics and faster recovery.
  • Despite advantages and cost-effectiveness, hesitations persist regarding UKA's use compared to total knee arthroplasty (TKA).
  • This study aimed to compare clinical outcomes, patient-reported outcomes (PROs), and satisfaction between medial UKA and TKA.

Purpose of the Study:

  • To evaluate and compare clinical and patient-reported outcomes (PROs) after medial unicompartmental knee arthroplasty (UKA) versus total knee arthroplasty (TKA).
  • To assess patient satisfaction levels following UKA in comparison to TKA.
  • To determine the safety and efficacy of UKA as an alternative to TKA for unicompartmental knee OA.

Main Methods:

  • A prospective, multi-center study included 116 patients with unicompartmental OA undergoing UKA.
  • Clinical results and PROs were assessed using the Knee Society Score (KSS) at 3 months, 1 year, and 2 years post-surgery.
  • A propensity score matched-pair analysis (116 pairs) compared UKA with TKA, matching for gender, age, BMI, and comorbidities. Pain and satisfaction were measured using a visual analog scale (VAS).

Main Results:

  • No revisions were reported in the UKA group within 2 years, while the TKA group had a 0.6% revision rate.
  • Both UKA and TKA groups showed significant improvement in PROMs post-surgery (p < 0.001).
  • The UKA cohort demonstrated significantly better Function-Score results (95 vs. 80, p < 0.001) and higher patient satisfaction (9.0 vs. 8.8, p = 0.019) compared to the TKA cohort at 12 months.

Conclusions:

  • Patients reported high satisfaction with both UKA and TKA.
  • UKA achieved superior function scores compared to TKA without an increased revision rate in the short-term follow-up.
  • UKA is a favorable treatment option for patients with unicompartmental knee OA.
