Summary

Patients with progressive multiple sclerosis (MS) have a higher likelihood of developing cervical spinal stenosis, particularly foraminal stenosis. This finding suggests increased vigilance for cervical spinal degenerative disease in MS patients.

Background:

  • Cervical spinal degenerative disease (SDD) can mimic progressive multiple sclerosis (MS) symptoms, leading to diagnostic overshadowing.
  • Cervical SDD involves root and cord irritation, potentially complicating MS diagnosis.

Purpose of the Study:

  • To investigate the hypothesis that cervical stenotic SDD is more prevalent in individuals with progressive MS compared to controls.
  • To assess the association between specific types of cervical SDD and progressive MS.

Main Methods:

  • A retrospective case-control study involving 111 participants (56 with progressive MS, 55 matched controls).
  • Magnetic resonance imaging (MRI) was used to evaluate five types of cervical SDD using established scales.
  • Multivariable regression analysis was employed to determine associations.

Main Results:

  • Moderate-to-severe cervical spinal degeneration was more frequent in the progressive MS group.
  • Foraminal stenosis was significantly more likely (OR 3.20) and severe in progressive MS patients.

  • Individuals with progressive MS exhibit increased susceptibility to foraminal stenosis.
  • A heightened clinical suspicion for cervical SDD is warranted in progressive MS patients presenting with relevant neurological symptoms.
  • Early identification of cervical SDD can guide appropriate management and monitoring in progressive MS.
