Summary

Hyperthyroidism may be linked to portal vein thrombosis. Thyroid function tests are recommended for patients with portal and mesenteric vein thrombosis when other causes are absent.

Background:

  • Non-malignant, non-cirrhotic portal and mesenteric vein thrombosis (PMVT) is uncommon.
  • Hyperthyroidism is associated with increased venous thrombosis risk due to altered coagulation and fibrinolytic factors.

Observation:

  • A 59-year-old male with Grave's disease presented with acute PMVT and hyperthyroidism.
  • Initial anticoagulation and antithyroid drug adjustment were performed.
  • All tests for PMVT causes were normal except for hyperthyroidism.

Findings:

  • Anticoagulation was discontinued post-thyroid normalization.
  • Recurrence of portal vein thrombosis occurred 3 years later, coinciding with hyperthyroidism.
  • Hyperthyroidism may be an underlying factor in PMVT.

Implications:

  • Hyperthyroidism should be considered in the differential diagnosis of PMVT.
  • Thyroid function testing is advised for unexplained PMVT cases.
  • Further research into the hyperthyroidism-PMVT link is warranted.
