Infantile dolichocolon, a colon anomaly, can cause severe constipation and bilateral hydronephrosis in neonates. Early intervention with enemas and lactulose effectively treated both conditions in a reported case.
Area of Science:
- Pediatric Gastroenterology
- Neonatal Urology
Background:
- Dolichocolon is a congenital colon anomaly characterized by redundant segments.
- It typically presents with constipation or volvulus in older individuals.
- This is the first reported case of dolichocolon presenting in infancy.
Observation:
- A 19-day-old neonate presented with severe constipation and abdominal discomfort.
- The infant developed pyelonephritis, leading to the discovery of bilateral hydroureteronephrosis via ultrasound.
- A barium enema confirmed the diagnosis of dolichocolon.
Findings:
- Infantile dolichocolon can manifest with severe constipation.
- Constipation associated with dolichocolon can lead to obstructive nephropathy.
- Treatment with enemas and lactulose resolved both constipation and hydronephrosis.
Implications:
- Neonatal severe constipation warrants evaluation for underlying causes like dolichocolon.
- Untreated dolichocolon-induced constipation may cause serious renal complications.
- Monitoring urinary flow is crucial for neonates presenting with severe constipation.

Proximal Convoluted Tubule (PCT):
The PCT is the initial segment of the renal tubule, extending from the Bowman's capsule that encloses the glomerulus. Its convoluted structure and microvilli-lined cells increase the surface area for reabsorption. The PCT reabsorbs glucose, amino acids, sodium, and water from the filtrate, ensuring essential...
