Background:

  • Optimal cerebral perfusion pressure (CPPopt) is a concept derived from the pressure reactivity index (PRx).
  • The COGiTATE trial is prospectively investigating CPPopt-guided therapy for severe traumatic brain injury (TBI).
  • Current evidence is lacking regarding factors influencing CPPopt availability (yield).

Purpose of the Study:

  • To examine the relationship between patient admission and treatment characteristics and the average CPPopt yield.
  • To identify potential predictors of CPPopt availability in severe TBI patients.

Main Methods:

  • Retrospective analysis of 230 severe TBI patients from the CENTER-TBI database.
  • Intracranial pressure (ICP) was measured using intraparenchymal probes.
  • CPPopt yield was calculated as the percentage of monitored time CPPopt was available, using the COGiTATE study algorithm.
  • Statistical analysis included age, GCS, gender, pupil response, hypoxia, hypotension, Marshall CT score, decompressive craniectomy, ISS, and 24-h TIL score.

Main Results:

  • The median CPPopt yield was 80.7% (IQR 70.9-87.4%).
  • No significant statistical correlation was found between the selected admission and treatment variables and CPPopt yield.

Conclusions:

  • In this multicenter retrospective study, selected patient admission and treatment variables were not associated with CPPopt yield.
  • Further investigation is required to determine factors influencing CPPopt availability in severe TBI.
