    The alternate rapid maxillary expansion and constriction (Alt-RAMEC) protocol significantly impacts dentoalveolar tissues, with changes comparable to standard rapid palatal expansion. This study evaluated these effects using cone-beam computed tomography.

    Background:

    • Maxillary hypoplasia often requires orthopedic intervention.
    • Alternate rapid maxillary expansion and constriction (Alt-RAMEC) is a technique used to expand the maxilla.
    • Understanding its effects on surrounding dentoalveolar structures is crucial.

    Purpose of the Study:

    • To evaluate dentoalveolar changes after the Alt-RAMEC protocol and facemask (FM) treatment.
    • To analyze immediate post-treatment effects using cone-beam computed tomography (CBCT).

    Main Methods:

    • Retrospective analysis of CBCT scans from 20 patients (mean age 9.64 years).
    • Measurements included dental/alveolar inclinations, buccal/palatal bone thickness, and height changes at three time points (T0, T1, T2).
    • Statistical analysis involved Shapiro-Wilks, paired t-tests, ANOVA, Wilcoxon signed-rank, and Friedman tests with Bonferroni correction.

    Main Results:

    • Buccal alveolar bone thickness and inclinations decreased significantly post-Alt-RAMEC (T1-T0) but stabilized after FM treatment (T2-T1).
    • Palatal bone thickness increased significantly after FM treatment (T2-T1).
    • Molar inclinations and alveolar bone height increased significantly post-Alt-RAMEC (T1-T0) and remained stable.

    Conclusions:

    • The Alt-RAMEC protocol induces dentoalveolar changes similar to those seen with conventional rapid palatal expansion.
    • The observed changes in bone thickness and inclination are significant but generally favorable for orthopedic goals.
